
Disability Program Coordinator

As a key contact, you will ensure seamless communication between participants, family members, support workers, and other stakeholders.
This role plays a crucial part in what makes us different from other support providers.
Key Responsibilities- Develop and maintain relationships between participants and their Hearth Support Workers team.
- Assess participant needs and develop our support program.
- Maintain relationships with support coordinators, house supervisors, and external stakeholders.
- Lead Support Worker teams to provide responsive, person-centered care.
- Report on client progress and prepare submissions for plan reviews.
- Coordinate weekly staff rosters and resolve rostering issues.
- Accurately document case notes and client reviews.
- Experience working in disability, preferably in a supervisory or managerial role.
- Strong administrative and business communication skills.
- Excellent time management and organisational skills.
- Client-centered approach with ability to advocate for participants.
- Strong interpersonal skills and resilience.
- Understanding of the NDIS and Microsoft Office suite.
