Care Partner

We are seeking a compassionate and experienced Care Partner to join our team. As a Care Partner, you will be the vital link between our clients and their care needs, ensuring older Australians receive the right care at the right time.

This is a dynamic leadership role where you will guide a team of passionate Home Care Workers to deliver high-quality, person-centred care that upholds dignity, independence, and quality of life.

As a Care Partner, you will assess client needs, develop and review care plans, coordinate services, advocate and educate clients, manage budgets, and build strong connections with clients, carers, and providers.

The key responsibilities include:

  • Assessing client needs and developing individualised care plans;
  • Coordinating services to meet client needs and preferences;
  • Advocating for clients and providing education and support to enhance their care experience;
  • Managing budgets and resources to deliver high-quality care services;
  • BUILDING STRONG CONNECTIONS WITH CLIENTS, CARERS, AND PROVIDERS TO DELIVER PERSON-CENTRED CARE.
