Clinical Care Professional Opportunity

We are seeking a highly skilled and compassionate Clinical Care Professional to join our team. As a valued member, you will provide exceptional clinical care and support services to individuals in the Home Care Package space.

Your primary responsibility will be to deliver high-quality care that meets the needs of our participants, while also ensuring alignment with relevant standards and regulations.

  • Function as a clinical partner in relation to the provision of home-based supports and packages.
  • Ensure the highest quality of care is provided to participants.
  • Ensure technologies, methods, and practices align with Aged Care Quality Standards, Charter of Rights, program requirements, and organisational policies.
  • Identify individual participant's needs and implement a clinical and/or general care plan in consultation with the participant, carer, and relevant stakeholders.
  • Develop clinical and general care plans based on goals, strengths, and identified needs in consultation with participants.
  • Manage workload effectively through excellent planning, organisational, and time management skills.
  • Achieve program performance goals by working with participants to manage their individual agreed services and budgets.
About You
  • Registered Nurse with current registration under the Australian Health Practitioner Regulation Agency (AHPRA).
  • Current unrestricted drivers' licence.
  • NDIS Worker screening clearance.
  • CPR Certificate (ongoing requirement for annual certification).
  • National Police Check.
  • Australian work rights.
  • Strong knowledge and practical experience working with people who are ageing from an enablement, restorative, and wellbeing perspective.
  • Confident and skilled clinician able to engage and motivate participants, families, staff, and stakeholders.
  • Strong case management experience (ideally in aged care) and case coordination.
  • Ability to conduct comprehensive assessments and develop goal-oriented care plans.
  • Ability to think creatively and use initiative, to work independently or as part of a multidisciplinary team.
  • Strong written and computer skills for the creation of plans, programs, reports, correspondence, and other documents.
  • Experience working with people who have dementia and their carers.
  • Experience and commitment to working with quality and risk management systems.
  • Capacity to provide education and hands-on learning in the workplace.
  • Experience working as part of a multidisciplinary team.
