We are seeking a skilled Clinical Assessor to join our Aged Care Assessment Services team. This is a 6-month fixed term position based on the Central Coast, offering flexibility and autonomy.
The successful candidate will conduct assessments to identify client needs, referring them to appropriate aged care services under the Aged Care Act. Key responsibilities include:
- Collaborating with clients, families, and multidisciplinary teams as required
- Conducting accurate aged care assessments using the Integrated Assessment Tool (IAT)
- Reviewing support plans to meet client needs
- Working effectively in a team to deliver key performance indicators within timeframes
To be successful in this role, you will require:
- Tertiary qualification in a health-related discipline
- Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA)
- Australian Driver's license and access to a registered, insured vehicle
- Minimum of one year experience delivering services in Aged Care settings or to Aged persons
