Transition Support Specialist

The Transition Support Specialist provides direct support to young people with complex needs, aged 15-21 years, who are at risk of homelessness as they transition to adulthood and post-care. This role focuses on developing protective independent living skills, family and support networks, and access to necessary specialist supports relevant to the young person's risks and needs.

Key Responsibilities:
  • Provide specialist transition support that identifies and responds to the complex support needs of high-risk young people based on best practice frameworks.
  • Promote and facilitate involvement of young people in goal setting and planning for their transition while supporting their connection to family, culture, and community.
  • Organise the provision of specialist services that address high-risk and complex needs of young people and assist with the development of independent living skills, enhancing capacity to maintain safe and stable housing, aid wellbeing, and post-care outcomes, and reducing the risk of homelessness.
  • Consult and develop relationships with government and non-government agencies to ensure efficient, effective, and financially supported service delivery.
  • Collaborate with regional line managers and senior program officers within a hub and spoke model ensuring continuous quality improvement and alignment with regional and programmatic priorities and identify opportunities for outcome improvement when supporting young people transitioning from care.
  • Use specialist transition practice knowledge to contribute to improvements in service delivery and enhance workforce capacity in child safety and external agencies.
  • Maintain sound, accountable, and legislatively compliant record keeping relevant to service delivery including contribution to programmatic data reporting systems.
