Support Coordinator Job Description

We empower individuals through tailored support that fosters independence and well-being. Our Support Coordinators play a vital role in helping participants navigate their NDIS plans to unlock their full potential.

The ideal candidate will be an experienced and enthusiastic individual who is passionate about continuing this vital work, providing amazing opportunities and a fantastic support network for our clients.

We are seeking highly organised and compassionate Support Coordinators to join our vibrant team, working with diverse participants, providers, and stakeholders to achieve all relevant KPIs, including billable hours.

Job Responsibilities
  • Build meaningful connections with people outside of funded support.
  • Guide participants as they implement their NDIS plans, ensuring informed choices are made.
  • Coordinate the implementation of supports, optimising capacity and funding management.
  • Support participants collect key evidence for changing needs and funding requests.
  • Explore opportunities in recreation, employment, housing, and more to enrich lives.
  • Ensure mainstream services step up and fulfill commitments.
  • Provide regular, insightful reports on progress and achievements.
Required Skills and Qualifications
  • Qualifications in social work, disability, mental health, or a related field.
  • Experience in NDIS support coordination, with in-depth knowledge of the NDIS and its range of services.
  • Confident in meeting KPIs, including billable hours.
  • Outstanding communication skills, with a natural ability to problem-solve, multi-task, and make decisions.
  • Eligible for or possess a Blue Card and NDIS Worker Screening Clearance.
