About the Transitional Nurse Practitioner Role

The role of the Transitional Nurse Practitioner (TNP) is to provide expert clinical support to patients transitioning to end-of-life care, establishing links with appropriate clinical services.

Key responsibilities include assessing, investigating and implementing treatment plans for patients presenting with palliative/end-of-life care needs. The TNP must have a strong understanding of advanced nursing practice guidelines and be able to work autonomously within the wider multidisciplinary team.

Requirements

  • Registered Nurse willing to enrol in a Master's program for Nurse Practitioner.
  • Minimum 5000 hours of advanced nursing/midwifery practice during the last six years, or working towards within the next three years.
  • Demonstrated clinical competency in Supportive and Palliative Care, with specific interest in aged care.
  • Ability to communicate effectively and negotiate with stakeholders.
  • Willingness to travel between sites and services.
