About the Role

We are seeking an experienced professional to manage a caseload of clients under approved funding. This involves maintaining budgets, onboarding new clients, and managing care requirements to maintain client independence.

Key Responsibilities:
  • Time Management: Prioritise tasks and manage time effectively to meet deadlines and deliver high-quality outcomes.
  • Aged Care Standards: Stay up-to-date with the latest aged care standards and guidelines to ensure compliance and best practice.
  • Liaising with Stakeholders: Build and maintain strong relationships with internal stakeholders, external networks, and clients to ensure seamless service delivery.

You bring:

  • Evidence of at least 1 year's experience in case management.
  • A relevant tertiary qualification in the community sector.
  • Demonstrated experience working with clients with complex needs, particularly in the aged, community, or disability sector.
  • A full driver's license and own reliable vehicle.
  • Knowledge of the assessment process.
  • A willingness to work in a team environment and contribute to a mission-based organisation.
  • Clearance (or willingness to obtain one).
