
Senior Community Mental Health Practitioner

The role involves providing nursing services to individuals in Northern Sydney Local Health Districts through triage, assessment, treatment, ongoing case management, and discharge planning.
Key Responsibilities:- Utilize extensive clinical experience in community mental health services to plan, implement, and evaluate care for complex mental health consumers.
- Proven ability in writing NDIS reports, mental health review tribunal reports, effective care planning, implementation, and evaluation of consumer goals, and liaison with community managed organisations.
Candidates Must Meet The Following Criteria:
- Current AHPRA registration as a Registered Nurse and current NSW Driver's licence.
