Role OverviewThe selected candidates will be part of the CO-ACT (Care of the Older Person and Community Transition) program at Hampstead Centre, working closely with a multidisciplinary team of consultants, nurses, and allied health professionals. Your primary responsibility will be to manage and oversee the treatment of patients within the service, ensuring high-quality care and contributing to Continuous Quality Improvement initiatives.
Key Responsibilities- Manage patient treatment plans, working collaboratively with the healthcare team to achieve optimal outcomes.
- Participate in multidisciplinary case conferences, sharing expertise and best practices to enhance patient care.
- Develop and implement individualized treatment plans, considering complex medical and social factors.
- Collaborate with geriatricians, general practitioners, and other healthcare professionals to ensure comprehensive patient care.
