We are seeking a Mental Health Case Manager for our Emotional Wellbeing for Older Persons Program.

This role involves providing case management services to elderly clients living in Residential Aged Care Facilities across the Central and Eastern Sydney PHN region with mental health needs.

The successful candidate will conduct comprehensive mental health assessments, develop personalised treatment plans and collaborate with healthcare professionals to ensure holistic care and support.

The role also includes advocating for the mental health needs of older adults within the RACF and engaging with staff to market and promote the EWOP program.

About the Role

This is a Maximum Term full-time role based at Summer Hill.

Key responsibilities include:

  • Conducting comprehensive mental health assessments for older adults
  • Developing personalised treatment plans tailored to each client's unique needs
  • Collaborating with healthcare professionals, caregivers, and family members to ensure holistic care and support
  • Advocating for the mental health needs of older adults within the RACF
  • Engaging with staff of RACFs to market and promote the EWOP program

Requirements

To be successful in this role, you will require:

  • A relevant tertiary qualification in social work, counselling, nursing, or psychology
  • A NSW Drivers licence and own vehicle
  • Experience working with people who have a mental illness and/or aged care
  • Excellent written and verbal communication skills

Benefits

Not-for-profit salary packaging benefits are available, reducing your taxable income by $15,900 plus an additional $2,650 for salary packaging accommodation, meals and entertainment.

Salary

$85,000 - $95,000 per annum depending on experience
