The Role

We are seeking a highly skilled Mental Health Practitioner to join our team in an exciting opportunity for the Emotional Wellbeing for Older Persons program. The successful candidate will work remotely, providing psychological wellbeing support services to residents in Residential Aged Care Facilities across Central and East Sydney PHN region.

In this role, you will be responsible for assessing and addressing the mental health needs of participants with mild and moderate conditions. You will also facilitate group work and workshops to promote best outcomes using various tools like counselling, mediation, and coaching.

Key Responsibilities

  • Collaborate with service providers, carers, and other organisations to ensure comprehensive support for participants.
  • Facilitate individualised care plans by engaging participants in goal-setting and capacity-building activities.
  • Engage with RACF staff to market and promote the EWOP program, encouraging their adoption and participation.

About You

  • You hold relevant tertiary qualifications in Psychology, Social Work, Counselling, or Registered Nursing, with current professional body membership.
  • You possess minimum 2 years of experience working with individuals with mental health challenges, preferably in the aged care sector.
  • You have excellent written and verbal communication skills, along with a valid NSW Driver's Licence and comprehensively insured vehicle.

Salary and Benefits

This permanent full-time position offers a competitive salary package, including not-for-profit salary packaging benefits, reducing your taxable income by up to $18,550 annually. You will also enjoy learning and development opportunities within the organisation.
