Eastern Health: A Leader in Mental Health Care
We are seeking an experienced and compassionate Senior Mental Health Social Worker to join our team. As a key member of our multidisciplinary team, you will be responsible for providing high-quality assessment, intensive case management services, and assertive outreach support to individuals experiencing mental illness.
About the Role
The primary focus of this role is to facilitate recovery-oriented care for people with lived experience of mental ill health. You will work collaboratively with Consultant Psychiatrists, Rehabilitation Teams, and other Senior Clinicians to ensure high-quality clinical standards and quality improvement initiatives.
Main Responsibilities:
- Clinical Case Management: Provide case management and support services to individuals with lived experience of mental ill health within the Behavioral Health Model Treatment Stream (BHMSTS).
- Collaborative Recovery Model: Work in partnership with individuals and their families/carers to provide high-quality assessment, intensive case management services, and assertive outreach support using the Collaborative Recovery Model.
- Leadership and Quality Improvement: Contribute to leadership and ensure high-quality clinical standards, quality improvement, and ongoing development of the Rehabilitation Stream.
About You
To succeed in this role, you will require:
- Experience supporting adults with lived experience of mental ill health.
- A qualification in Social Work approved by the Australian Association of Social Workers (AASW) for membership as a social worker.
- Ability to work autonomously and as part of a diverse team.
- Commitment to supervised practice, ongoing professional development, and further developing skills in clinical mental health.
- Strong commitment to high-quality care.
- Highly developed interpersonal and leadership experience.
- Excellent communication and organisational skills.
- A valid driver's license.
What We Offer
