">Role Overview:

This challenging and rewarding role involves providing clinical expertise and leadership to deliver high-quality care to older people presenting to the Emergency Department or living within Residential Aged Care Facilities.

">Main Responsibilities:
  • Advanced Practice Nursing: Utilise a multidisciplinary approach to screening, triage, assessment, care coordination, and care planning for older people.
  • Leadership and Management: Lead and manage the RASS/GEDI team to ensure safe practice and high-quality outcomes.
  • Evidence-Based Practice: Develop and implement evidence-based best practice knowledge and skills to improve patient outcomes.
  • Collaboration and Communication: Collaborate with patients, families, and the broader healthcare team to ensure individualised care plans are implemented.

  • Nursing Professional: Demonstrated ability to be an effective nursing professional with capability for high-level engagement across stakeholders.
