Support Coordinator

This role offers the opportunity to make a positive impact in the lives of individuals with disabilities by providing high-quality support services.

The Support Coordinator will be part of a dynamic team that promotes independence and inclusion in the community.

About the Role

The Support Coordinator will provide respectful and dignified personal care and daily living support to clients, while also promoting their participation in outdoor and hands-on activities such as sports, art, music, gardening, or cooking.

Key Responsibilities:

  • Provide individualized support plans to enable clients to engage in recreational activities.
  • Promote social inclusion by facilitating connections with families, colleagues, and other service providers.
  • Maintain accurate records of client progress and communicate proactively with team members and supervisors to report any concerns.

Requirements

To be successful in this role, you must have or be willing to obtain the following:

  • Current Level 1 First Aid & CPR Certificate.
