About the Role

This is an exciting opportunity to join our dynamic Allied Health and Nursing team as a Comprehensive Home Healthcare Professional. As a valued member of our team, you will work collaboratively with clients, families, and healthcare professionals to deliver comprehensive assessments, care planning, and case management services in the client's home.

Key Responsibilities
  • To conduct comprehensive assessments in the client's home, identifying their needs and developing personalized care plans to promote optimal health outcomes.
  • To provide skilled nursing interventions, including medication management, wound care, and mobility assistance, as required.
  • To collaborate with multidisciplinary teams, including doctors, therapists, and other healthcare professionals, to ensure seamless care coordination.
  • To maintain accurate and up-to-date records of client interactions, progress, and care plans.
Requirements

We are seeking applicants who possess:

  • A current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse/Midwife.
  • Demonstrated experience in community nursing, aged care, or rehabilitation settings.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work autonomously and as part of a collaborative team.
  • Proficiency in using health information systems and data collection tools.
