Community Care Specialist

We are seeking a skilled Community Nurse to deliver holistic, person-centred care to older adults experiencing mental health challenges, chronic conditions, and complex needs.

This role involves providing direct clinical nursing care to clients in their homes, conducting assessments, developing care plans, and ensuring safe, quality practice.

  • Assessing client needs and developing individualized care plans
  • Providing wound care, palliative care, education, and support
  • Liaising with healthcare professionals to maintain comprehensive care
  • Maintaining accurate documentation and records
Requirements

To be successful in this role, you will need:

  • A current AHPRA registration following completion of a Bachelor of Nursing or similar qualification
  • Minimum 3 years post-graduate nursing experience, including clinical assessments and community care
  • Strong initiative and ability to work independently
  • Excellent computer skills and the ability to adapt to new systems
  • NSW Drivers license and reliable vehicle
