Comprehensive Care Specialist

  • Key Responsibilities:
  • Conduct comprehensive assessments of older adults' physical and emotional needs.
  • Develop and implement personalized care plans that address the unique needs of each individual.
  • Collaborate with multidisciplinary teams to ensure seamless coordination of care and optimize patient outcomes.
  • Utilize in-depth knowledge of My Aged Care and related programs to assess and support routine and complex aged care presentations.
  • Maintain accurate and detailed records of patient interactions, assessments, and care plans.
  • Participate in ongoing professional development to stay up-to-date with best practices and emerging trends in aged care.
