Aged Care Clinical Assessor Job Description

We are seeking an experienced and compassionate Aged Care Clinical Assessor to join our team. As a key member of our assessment service, you will play a crucial role in identifying the needs of older adults and connecting them with essential services that support independent living.

This rewarding role involves conducting thorough assessments to identify the needs of older adults, facilitating referrals to relevant services, promoting functionality, and engaging with stakeholders. You will work closely with families and carers, recognizing them as vital partners in delivering high-quality care.

Responsibilities:
  • Conduct comprehensive assessments to identify the needs of older adults.
  • Foster collaboration with multidisciplinary teams to deliver seamless care.
  • Promote independence and quality of life for older adults through proactive support and referrals.
  • Engage with families and carers to ensure their involvement in care planning and delivery.
Qualifications and Skills:
  • Minimum 1 year of experience in aged care, including client assessments and/or service delivery.
  • Tertiary education in a health-related field (e.g., nursing, occupational therapy, physiotherapy, social work).
  • Current unrestricted General Registration with AHPRA or relevant professional association.
  • Proficiency in Microsoft Office and client management systems.
  • Valid Australian driver's license and ability to travel within your designated area.
