Home Care Coordinator

We are seeking an experienced professional to join our team as a Home Care Coordinator. This is a full-time position, working Monday to Friday, 8:30 AM – 4:30 PM.

  • This role involves conducting client care assessments and overseeing ongoing care planning.
  • Maintaining client compliance and managing a caseload of clients is also a key responsibility.
  • The successful candidate will be responsible for coordinating and delivering personalised care aligned with client needs, care plans, and allocated resources.
  • General administrative tasks related to the role will also be required.

  • A relevant qualification (Cert IV or Diploma in Nursing highly regarded) is essential for this role.
  • Experience in aged care, NDIS, or mental health case management (preferred) is also desirable.
  • The ideal candidate will have knowledge of My Aged Care, Home Care Packages, and NDIS frameworks.
  • A proven track record in assessing and planning community-based care is also necessary.
  • Proficiency in Microsoft Office 365, SharePoint, Outlook, and rostering software is required.
  • Excellent verbal and written communication skills with strong customer service abilities are also essential.
  • The ability to handle incoming calls professionally and courteously is also required.
  • Strong organisational, problem-solving, and decision-making skills are also necessary.
  • A collaborative team player with a proactive attitude is also desired.
  • Ability to build strong relationships with external organisations for client referrals is also required.
  • Commitment to confidentiality and privacy in line with company policies is also essential.
