Job Title: Care and Support Specialist

About the Role:

We are seeking a proactive, compassionate, and forward-thinking professional to join our team and play a vital role in delivering high-quality, person-centred care across the region.

This is an opportunity to step beyond traditional case management and into leadership in action. You will work from our office and have use of a company vehicle for all client-related travel.

Key Responsibilities:
  • Deliver person-centred case management tailored to each client's goals and needs
  • Prepare and review budgets with clients to align with their care objectives
  • Manage a caseload, conducting regular assessments and reviews
  • Build and maintain strong, trusting relationships with clients and families
  • Coordinate services across allied health, nursing, and home support
About You:

You will thrive in this role if you are a natural coordinator with strong problem-solving and advocacy skills, and a genuine passion for improving the lives of older Australians.

Required Skills and Qualifications:
  • Previous experience in aged care case management (HCP/CHSP preferred)
  • Background in community or aged care service coordination
  • Excellent planning, communication, and interpersonal skills
  • Able to work independently while contributing to a multidisciplinary team
  • A valid Australian driver's licence and clean driving record
