Job Description:

Deliver exceptional care and support to individuals in their own homes, empowering them to maintain their independence and live with dignity.

As a skilled Home Care Coordinator, you will conduct thorough assessments of client needs, develop tailored care plans, and oversee the delivery of high-quality services that meet the unique requirements of each individual.

Required Skills and Qualifications:
  • Relevant qualifications (Cert IV or Diploma in Nursing highly regarded)
  • Experience in aged care, NDIS, or mental health case management (preferred)
  • Knowledge of My Aged Care, Home Care Packages, and NDIS frameworks
  • Proven ability to assess and plan community-based care
  • Proficiency in Microsoft Office 365, SharePoint, Outlook, and rostering software
  • Excellent communication and interpersonal skills
  • Strong organisational and problem-solving abilities
  • Collaborative team player with a proactive attitude
