We are seeking a highly skilled and compassionate individual to fill the role of NDIS Case Manager. In this pivotal position, you will play a vital role in coordinating and overseeing individualised support services, ensuring participants receive the highest standard of care.

Key Responsibilities:

The ideal candidate will possess excellent case management skills, with the ability to develop and implement individualised care plans, conduct thorough assessments, and maintain strong relationships with stakeholders.

  • Comprehensive Case Management: Coordinate and manage support services, aligning them with NDIS practice standards and participant care plans.
  • Intake and Initial Engagement: Oversee the intake process for new participants, including referrals, screening, risk assessments, and resource evaluations.
  • Personalised Care Planning: Develop and implement individualised care plans, considering participants' goals, needs, and preferences.
  • Participant Assessments and Reviews: Conduct initial and ongoing assessments, monitor progress towards goals, and adjust support strategies as needed.
  • Stakeholder Engagement: Foster strong, collaborative relationships with participants, their families, support workers, and allied health professionals.
  • Team Leadership and Support: Provide guidance and support to colleagues on complex issues, risk management, and incident resolution.
  • Compliance and Documentation: Ensure accurate and timely documentation of participant information, adhering to organisational policies and NDIS requirements.
  • Crisis and Risk Management: Actively manage crisis situations, participant risks, and barriers to effective service delivery, prioritising safety and quality of care.

Qualifications & Experience:

Applicants should possess:

  • A relevant qualification in social work, psychology, social sciences, health, or a related field.
  • Minimum 2 years' experience in disability service management or case management, preferably within the NDIS framework.
  • Demonstrated understanding of NDIS Practice Standards, participant support needs, and case management principles.
  • Exceptional written and verbal communication skills, with the ability to engage effectively with diverse stakeholders.
  • Strong leadership, mentoring, and problem-solving skills, with attention to detail and a proactive approach.
  • Proficient in Microsoft Office and capable of managing and reporting on participant care data.
