Job Summary:

The Senior Client Service Manager will be responsible for managing a caseload of community-based older Australians requiring government-funded aged care services in their homes.

Main Responsibilities:
  • Manage clients' service plans to ensure each client's needs are met within program guidelines.
  • Assess clients' requirements and develop tailored care strategies.
  • Collaborate with our Clinical, Scheduling, and Service Delivery teams to deliver comprehensive support services.
Key Requirements:
  • Experience in community-based case management is highly desirable.
  • Excellent communication skills, both verbal and written.
  • Demonstrated empathy and compassion.
  • Strong problem-solving abilities and critical thinking skills.
  • Cultural competence and ability to work effectively with diverse populations.
