At our organisation, we are seeking a skilled and compassionate Community Health Professional to join our team.
This is an exciting opportunity for a Registered Nurse with experience in community care to make a real difference in the lives of our clients. As a Community Health Professional, you will be responsible for providing high-quality nursing care to clients in their own homes, as part of our Home Care Package Program and National Disability Insurance Scheme.
Key Responsibilities:
- Providing comprehensive nursing assessments to support clients accessing services and support.
- Working collaboratively to develop and implement individual care plans for complex clients.
- Supporting CatholicCare's Clinical Governance Framework by providing expert advice to other staff regarding care and incident response for clients with complex needs.
- Proactively contributing to the development and implementation of policies and procedures to enhance the services provided to all clients.
- Providing leadership for the nursing service within the identified region/s and supporting and overseeing other enrolled nurses providing in-home nursing care.
Requirements:
- Bachelor of Nursing Degree.
- Minimum 5 years of nursing experience.
- Minimum 3 years community (or aged care) Nursing experience (Desirable).
- Current C class Driver's license (QLD).
In addition to the above, you will need to demonstrate:
- Ability to assess client needs and develop appropriate plans for interventions and treatment for clients.
- Ability to develop and provide reports regarding clients' needs and conditions.
- Ability to professionally work with internal and external stakeholders to meet client needs.
