About the Responsibilities

  • Engage with clients and their families to assess their needs and establish trusting relationships.
  • Conduct comprehensive assessments and develop individualized care plans.
  • Provide nursing and in-home care services, including wound care, insulin administration, and medication management.
  • Address inquiries and provide detailed feedback as required.
  • Ensure all client documentation is completed accurately within our systems.
  • Interpret and communicate relevant policies and procedures regarding health, aged care, and disability services to staff.
  • Ensure compliance with local, state, and federal regulations related to aged and disability care services.
  • Foster best practice standards in caregiver management and encourage ongoing professional development.
  • Coordinate with our operational teams to ensure accurate billing in accordance with care plans.
  • Ensure adherence to Occupational Health and Safety (OH&S) regulations.

About You

  • A Enrolled Nurse (EN) with 1-2 years of experience in community nursing and current AHPRA registration.
  • Alternatively, a Certificate III or IV in Aged/Disability Care with a minimum of 4 years' experience.
  • Proven experience in aged care, disability, or mental health within a community setting.
  • A strong understanding of Aged Care and Disability Standards, as well as consumer-directed care principles.
  • Current First Aid, CPR certification, and National Police Check.
  • A valid driver's license and access to a reliable car with comprehensive insurance.
  • High level of computer literacy and proficiency with relevant software.
  • A positive, 'can-do' attitude with the ability to work effectively in a collaborative team environment.
