Job Description

We are seeking a highly skilled Family Support Specialist to join our team. In this role, you will work with vulnerable families to provide intensive support and enable children to live safely at home.

About the Role

The Family Resilience program is a therapeutic program designed to provide 12-month in-home support to families with complex needs. As a Family Support Specialist, you will be responsible for engaging clients, conducting comprehensive assessments, developing client plans, and coordinating services to ensure effective delivery.

Key Responsibilities

  • Conduct thorough assessments of clients' needs and develop personalized plans to address areas of risk and vulnerability.
  • Work collaboratively with clients, other service providers, and stakeholders to ensure seamless service delivery.
  • Provide wrap-around services to clients as needed, including referrals to external agencies and resources.
  • Monitor and review client progress, making adjustments to plans as necessary to achieve optimal outcomes.
