Our Better Connect program is a comprehensive care service addressing service gaps by enhancing accessibility and integrating primary mental health care, psychosocial support, and early intervention.

We focus on the 'missing middle' in our community, ensuring sustainable, adaptable services responsive to local needs.

The consortium includes multiple partners, each contributing expertise to deliver holistic wellbeing solutions.

Our team aims to promote collaboration among providers, foster a culture of recovery, and ensure accessible services for all.

In this role, you will contribute to high-quality clinical care coordination as part of our multidisciplinary consortia team. You will work closely with clients and families to provide evidence-based interventions and support tailored to individual needs.

This is a unique opportunity to work collaboratively in a dynamic environment, sharing knowledge and best practices to drive positive change in the lives of those we serve.

You will be responsible for:

  • Addressing service gaps by enhancing accessibility and integrating primary mental health care, psychosocial support, and early intervention.
  • Promoting holistic wellbeing by considering broader determinants of health, fostering collaboration among providers, and ensuring a sustainable, adaptable service responsive to community needs.

To succeed in this role, you will need to possess strong communication skills, excellent interpersonal abilities, and a commitment to delivering high-quality care.

You should also have experience working with electronic client/customer management systems and Microsoft Office applications.
