About the Role:

This is an opportunity to make a positive impact in mental health service delivery as a Clinical Care Coordinator.

The successful candidate will contribute high-quality evidence-based clinical care as a member of a multidisciplinary team delivering care coordination services for children and youth.

Key responsibilities include addressing service gaps, promoting holistic wellbeing, and collaborating with providers to ensure a sustainable, adaptable service responsive to community needs.

This part-time role offers flexibility to work 4 days per week or a 9 day fortnight, with the option to work from home one day per week.

As a valued member of our team, you will have the opportunity to work collaboratively with healthcare professionals and stakeholders to enhance service delivery through comprehensive, community-focused care.

What We're Looking For:
  • AHPRA registration as an Occupational Therapist or Mental Health Nurse, General or Provisional AHPRA registration as a Psychologist/Clinical Psychologist, or AASW registration/eligibility as a Social Worker.
  • Knowledge and understanding of theory underpinning evidence-based interventions and clinical practice in mental health and alcohol and drug use.
  • Highly developed communication and interpersonal skills, with the ability to engage with people, advocate on their behalf, and establish effective relationships.
