Home Care Clinical or Non-Clinical Care Partner Job Description

Our organization is seeking a skilled and compassionate Home Care Clinical or Non-Clinical Care Partner to join our team.

  • Undertake assessments of participants in their homes, ensuring follow-up to reduce risks and evidencing the care provided.
  • Ongoing case management supporting participants in clinical and holistic needs, monitoring and communicating on wellbeing and reporting progress.
  • Document participant goals, progress, and outcomes using person-centered, wellness, reablement, and ablement strategies.
  • Administrative duties including data entry, processing invoices, following up participant enquiries, and documenting all interactions to ensure compliance.
  • Ensure evidence is gathered and uploaded, document all evidence requirements in line with Compliance, and oversee participant budgets.
  • Work with medical practitioners and Allied Health professionals, ensuring services are provided in a caring and respectful way according to policy and procedures.
  • Provide training, coaching, and guidance to Community Support Professionals, Enrolled Nurses, and Support Staff/Care Partner colleagues as required.
  • Conduct skills assessments of Community Support Professionals performing complex care services as required.
