The role of Clinical Care Coordinator is a vital part of delivering high-quality, evidence-based care as a member of a multidisciplinary team. This position plays a key role in enhancing the accessibility and integration of mental health care, psychosocial support, and early intervention for children and youth.

Responsibilities:
  • Provide high-quality, evidence-based clinical care as part of a multidisciplinary team.
  • Address service gaps by enhancing accessibility and integrating primary mental health care, psychosocial support, and early intervention.
  • Promote holistic wellbeing by considering broader determinants of health and fostering collaboration among providers.
Requirements:
  • Must have one of the following: Full registration as an Occupational Therapist or Mental Health Nurse, General or Provisional registration as a Psychologist/Clinical Psychologist, OR, AASW registration/eligibility as a Social Worker, OR, Level 2 ACA registration/eligibility.
  • Knowledge and understanding of evidence-based interventions and clinical practice in mental health and alcohol and drug use.
  • Highly developed communication and interpersonal skills.
  • Ability to engage with people, advocate on their behalf, and establish effective relationships with staff, clients, families, and stakeholders.
  • Experience with electronic client/customer management systems and Microsoft Office applications.
  • Eligibility to apply for NDIS worker screening clearance and hold a Blue Card for paid employment.
