Regional Care Specialist

The Regional Care Specialist plays a crucial role in delivering high-quality care to clients with diverse needs due to aging, acquired or inherited disabilities.

Our organization provides personalized support services to clients in their homes, ensuring they receive the best possible care and services required.

Key Responsibilities:

  • Assessing client needs and developing individualized care plans
  • Coordinating with healthcare professionals and service providers to ensure seamless care delivery
  • Providing ongoing support and guidance to clients and their families
  • Collaborating with internal teams to ensure efficient care coordination and resource allocation
  • Staying up-to-date on industry developments and best practices to enhance care quality
  • Managing client budgets and preparing reports for funding and resources
  • Working closely with clients to promote independence and rehabilitation
  • Establishing strong relationships with community partners and organizations
  • Participating in performance management and development initiatives

Required Skills and Qualifications:

  • Diploma of Community Services - Case Management
  • Cert IV in Aged Care or Disability Care
  • Minimum 7 years' experience working in the Aged Care or Disability sector
  • Demonstrated knowledge and previous experience as a Care Coordinator or similar role
  • Minimum 5 years' experience in team leadership/management positions
  • Current Police Check
  • Current First aid Certificate
  • Working Knowledge of Regulations, standards, frameworks, and codes
  • Up-to-date immunization
