Job Summary: We are seeking a Registered Nurse to join our interdisciplinary team as a direct supervisor. The role involves providing participant-centered care, assessment, screening, and education for individuals accessing SAFER services.

Key responsibilities include:

  • Providing care and support for individuals with high levels of poly-substance use and various acute and chronic conditions.
  • Collaborating with the SAFER team to support individuals contemplating or accessing prescribed pharmaceutical alternatives.
  • Delivering care, assessment, screening, education, referrals, and advocacy regarding substance use, health concerns, mental health, sexual health, wound management, and general wellness.
  • Developing individualized care plans in collaboration with participants and the SAFER team.
  • Conducting screenings and assessments, administering medications as ordered, and providing education and referrals to manage health and navigate services.
  • Participating in staff meetings, trainings, and community building activities.
