We are seeking a skilled Aboriginal Case Manager to support individuals experiencing homelessness or at risk of homelessness.

Key Responsibilities:

  • Provide culturally appropriate case management and support services to First Nations people and their families.
  • Develop strong relationships with clients to understand their needs and goals, and provide tailored support to help them achieve stability.
  • Work collaboratively with internal teams, external service providers, and community organisations to facilitate access to housing, healthcare, employment opportunities, and other essential resources.
  • Advocate on behalf of clients to ensure they receive fair and equitable treatment when accessing services and support.
  • Conduct comprehensive assessments of clients' needs and develop individualised care plans with achievable goals that empower clients towards self-sufficiency.
  • Maintain accurate and up-to-date case notes, reports, and client records in accordance with organisational guidelines.
  • Participate in regular supervision sessions, team meetings, training programs, and professional development activities to enhance skills and knowledge relevant to the role.

About You:

  • Cert IV Community Service and or other relevant qualification and or case work experience in an associated field
  • Current Driver's Licence
  • Satisfactory Criminal record check
  • Current Working with Children Check (NSW)
  • Understanding and experience of working with Aboriginal people and their communities
  • Demonstrated experience in case planning and coordination of people with complex support needs
  • Understanding of, and demonstrated ability to work with Human Service Agencies
  • Demonstrated experience and or understanding of Trauma Informed Care
  • Excellent communication skills, particularly in the areas of negotiation, facilitation, mediation and problem solving
  • Intermediate computer literacy skills including Microsoft office suite and databases
