About the Role

We are seeking a dedicated and experienced Registered Nurse to join our Home Care team in the Northern Territory, supporting our Darwin community.

This role is responsible for delivering high-quality clinical and care coordination services to achieve person-centered care for clients/care recipients that is responsive to their assessed needs.

Key Responsibilities
  • Conduct client on-boardings, reviews, assessments, and develop budgets for clients, resulting in clients having a current and holistic assessment and a service plan that manages all risks associated with their assessed needs, within their service budget, and with a health-promoting focus.
  • Deliver nursing services to the client in their home or via telehealth, which is best practice and maximizes their ability to continue to live in the community.
  • Ensure effective management of clinical matters within the region.
  • Provide effective leadership to direct care staff in the region to meet client/care recipient needs in a timely and effective manner.
  • Ensure all assessments and documentation meet legislative requirements.
  • Monitor staff in the region, ensuring the required levels of competency is achieved, including conducting Home Care Assistant competency assessments.
  • Conduct orientation and training for staff within the region, under the direction of the Care Manager - Home Care.

The successful candidate will have the following skills and experience:

  • Bachelor of Nursing, or recognized equivalent, and current registration and practicing certificate with AHPRA as a Registered Nurse.
  • Current and unrestricted driver's license and own registered and insured car.
  • Effective decision-making ability, including the ability to identify potential risks and escalate as appropriate within the organization.
  • Strong organizational and time management skills.
  • Excellent interpersonal and communication skills, as well as the ability to build positive relationships with clients and their representatives.
  • Demonstrated competence in auditing and quality assurance activities.
  • A basic knowledge of the requirements of the Aged Care Standards as applicable to the role.
  • You will be suited to this role if you have a positive mindset and can prioritize well.
  • Ability to demonstrate integrity, calmness, and personal resilience.
  • Passionate about person-centered care, enabling clients to maintain maximum independence, you will be able to seek creative solutions in your approaches to overcome any barriers.
