About the Role

We are seeking a highly skilled and compassionate Clinical Care Coordinator to join our team at Open Minds. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality mental health services to individuals in need.

Key Responsibilities
  • Conduct comprehensive clinical assessments to identify client needs and develop personalized treatment plans.
  • Deliver evidence-based psychological interventions and manage a clinical caseload with a focus on client-centered care.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and transitions.
  • Perform risk assessments and provide support to clients and their families.
  • Uphold confidentiality and ethical standards in all aspects of your work.
Requirements
  • Formal tertiary qualification in psychology, social work, mental health nursing, or a relevant discipline.
  • Current annual practicing certificate from AHPRA or eligibility to register with the Australian Association of Social Workers.
  • Demonstrated excellence in mental health service delivery, with significant experience working with complex mental health presentations.
  • Experience in cross-cultural settings, especially with First Nations People, and in trauma-informed, recovery-oriented services.
  • Strong stakeholder engagement, organisational, communication, problem-solving, and analytical skills.
