Dynamic Care Coordinator

We seek a seasoned Clinical Support and Development Nurse to drive quality improvement initiatives and contribute to creating a culture of continuous learning and growth within our multidisciplinary team.

Key Responsibilities:
  • Collaborate with healthcare professionals to develop and implement personalized care plans for patients with complex medical, functional, cognitive, and psychosocial needs.
  • Lead the development of preceptors and provide support to new team members and graduate nurses in enhancing their clinical skills and knowledge.
Essential Qualifications:
  • Proven experience in subacute, geriatric, or dementia care nursing
  • Strong passion for clinical leadership, quality improvement, education, and mentoring
  • Excellent communication and interpersonal skills
  • Commitment to delivering person-centred care through effective collaboration with the multidisciplinary team
