">We are seeking a compassionate and experienced mental health care coordinator to provide direct, person-centred support to individuals with mental health challenges.
">As a mental health care coordinator, you will be responsible for providing practical and emotional support to individuals experiencing mental health difficulties. You will work closely with clients to help them manage their conditions, build coping strategies, and achieve their personal goals.

- Assist clients in daily living activities, including personal care, social interactions, and household tasks, where necessary.">
- Help clients develop and implement individual care plans that promote well-being and recovery.">
- Monitor clients' mental and physical health, documenting progress and any concerns.">
- Support clients in accessing community services and maintaining appointments (e.g., with therapists, doctors, or community support groups).">
- Promote independence and empowerment, encouraging clients to make informed decisions about their care and treatment.">
- Work closely with family members, carers, and other professionals to ensure comprehensive care coordination.">
- Participate in regular supervision and team meetings, contributing to ongoing development and training opportunities.">
Required Skills and Qualifications:

- Experience: Minimum of 2 years of experience working in mental health support, either in a community, residential, or clinical setting.">
- Qualification: A relevant qualification in Mental Health, Social Work, Health and Social Care, or a related field (e.g., Diploma in Health and Social Care, Master of Psychology (Clinical) or Diploma in Counselling etc).">
- Skills: Strong communication, problem-solving, and interpersonal skills. Ability to work with individuals from diverse backgrounds and with a range of mental health conditions.">
- Compassionate and Patient: Empathy and respect for individuals living with mental health conditions are essential, along with the ability to maintain professional boundaries.">
- Team Player: Ability to collaborate with colleagues and external professionals to provide holistic care.">
- Flexibility: Ability to work a variety of shifts, including evenings, weekends, and holidays if necessary.">
