About the Role: Support Coordinator

As a Support Coordinator for Ageing Communities, you will play a vital role in providing personalised 1:1 Home Care support to consumers accessing our Aged Care Home Care services. Our consumers in these programs receive ongoing care, during which time you will support their reablement and improved quality of life.

You will work closely with our consumers, their families, and other healthcare professionals to develop and implement individualised care plans that promote independence, choice, and control. Your primary focus will be on ensuring that our consumers receive the highest level of care and support to live safely and comfortably in their own homes.

The ideal candidate will possess strong communication and interpersonal skills, with the ability to build rapport with our consumers, their families, and other stakeholders. You will also have a passion for delivering high-quality care and support, with a focus on promoting dignity, respect, and inclusivity.

About You:

  • Minimum Certificate III in Individual Support (Disability, Ageing or Community);
  • A current First Aid Certificate and CPR Certification;
  • Computer savvy: have a smartphone, computer, or tablet and ability to document accurate notes about all support shifts;
  • An Australian Driver's Licence and ownership of a registered vehicle with comprehensive car insurance;
  • Strong communication skills with the ability to build and maintain positive relationships;
  • Passionate about making a positive impact in our ageing community;
  • Rights to work in Australia;
  • Fully vaccinated for COVID-19 with a minimum of 2 doses;

Salary Package:

We offer a competitive salary package, including a minimum hourly rate of $33.41 plus superannuation. Depending on your location and qualifications, you may also be eligible for additional benefits such as travel allowance and meal entertainment expenses.
