NDIS Support Coordinator Opportunity

Bodewell Community Care is seeking a highly skilled and dedicated NDIS Support Coordinator to join our team. As a key member of our team, you will play a vital role in ensuring our participants receive the support they need to live independently in their homes.

About the Role:

  • Full-time position, Monday to Friday, 9 am to 5 pm, 76 hours per fortnight
  • Company vehicle available for work use
  • Laptop, mobile, equipment, and uniforms provided
  • Ongoing training and development opportunities
  • Staff Employee Assistance Program
  • Willing to use own vehicle when required (km paid)

Key Responsibilities:

  • Link participants with directed care supports
  • Engage in assessment and goal planning
  • Create referrals to appropriate services
  • Ongoing monitoring of budgets and participation in plan reviews
  • Follow operational guidelines and NDIS legislation and procedures

Requirements:

  • Tertiary qualification in nursing, allied health, social work, or human services
  • Working with Children Check Clearance
  • NDIS Clearance or willingness to obtain
  • Valid driver's license and reliable vehicle
  • Previous experience in NDIS or relevant programs
  • Demonstrated experience in managing client budgets
  • Essential to have previous experience working in the aged care and/or disability field, ideally as a care manager/support coordinator
  • Experienced in assessment and record keeping via databases and/or client management systems
  • Able to work autonomously and as part of a team
  • Able to manage conflicting priorities – excellent time management skills required
  • Ability to work within organisational policies and procedures and legislative requirements
  • Level 3 Support Coordination experience preferred
