About the Role

The Aboriginal Care Navigator is a key position within the Medicare Mental Health Centre Wagga Wagga, working collaboratively with consumers to connect them with services and supports in the community.

This role delivers practical support through information sharing and education, identifying and addressing barriers to access and facilitating warm referrals to ensure consumers are linked to the right service at the right time.

The Aboriginal Care Navigator works closely with the Medicare Mental Health Centre Wagga Wagga team and external service providers to maximise care outcomes for Aboriginal consumers.

Key Responsibilities
  1. Engage with Aboriginal consumers and their families or carers to provide care navigation and referral support.
  2. Identify and address barriers to access and facilitate warm referrals to ensure consumers are linked to the right service at the right time.
  3. Work collaboratively with the Medicare Mental Health Centre Wagga Wagga team and external service providers to maximise care outcomes for Aboriginal consumers.
Requirements

To be successful in this role, you will need:

  1. To identify as an Aboriginal and/or Torres Strait Islander and be an active member within the Aboriginal communities of Wagga Wagga and surrounding areas.
  2. A tertiary qualification in a relevant discipline, and/or extensive experience in Aboriginal health, AOD, mental health, community services or social sciences.
  3. Demonstrated ability to engage with Aboriginal consumers and their families or carers to provide care navigation and referral support.
  4. Demonstrated understanding of supports and services available in the local and regional communities.
