About the Role

We are seeking a dedicated and compassionate Care Finder to join our team at Marathon Health opportunities.

As a Care Finder, you will play a vital role in providing specialized support to vulnerable older people, helping them navigate and access aged care services.

Key Responsibilities:

  • Provide intensive assistance to help individuals access aged care services
  • Break down barriers for recipients, connecting them with vital community supports
  • Improve integration between health, aged care, and other local systems
  • Engage with older individuals facing significant challenges in accessing aged care services
  • Collaborate and communicate effectively with participants, their families, and relevant stakeholders

About You

We are looking for a candidate with qualifications in Allied Health, Nursing, Community Services, Social Work, or equivalent.

You will have a solid understanding of the aging process and related life transitions, as well as knowledge of local and regional aged care supports and services.

Key Requirements:

  • Qualifications in Allied Health, Nursing, Community Services, Social Work, or equivalent
  • Solid understanding of the aging process and related life transitions
  • Knowledge of local and regional aged care supports and services
  • Commitment to a person-centred approach that respects individual needs and experiences
  • Well-developed communication and interpersonal skills, with the ability to connect with diverse backgrounds
  • Strong problem-solving abilities and a solutions-focused mindset
  • Proficient administrative skills, including experience with client management systems and databases
  • A current Australian driver's license and a willingness to travel occasionally for work
