Healthcare Navigator

Role Overview

Marathon Health opportunities is seeking a dedicated Healthcare Navigator to join our team in Wagga Wagga, NSW (Wiradjuri Country) on a part-time basis until June 2025.

In this role, you will collaborate with our care coordinators to address the diverse social needs that influence health outcomes. You will take a holistic approach, helping individuals overcome barriers related to finances, transportation, housing, legal issues, and social isolation.

Key Responsibilities

• Advocate for and connect participants with local services (aged care, NDIS, community services) to help them achieve their health and wellbeing goals.
• Work collaboratively with participants and their families to create tailored support plans.
• Identify and address the wider determinants of health—social, economic, and environmental factors—and how they impact communities.
• Build strong partnerships across diverse organisations to deliver the best outcomes.
• Communicate effectively with people from culturally and linguistically diverse backgrounds and First Nations communities.

Selection Criteria

• Qualifications in Community Services, Human Services, Social Work, or a Health-related field.
• Experience advocating for and connecting participants with local services.
• Ability to work collaboratively with participants and their families.
• Understanding of the wider determinants of health.
• Strong communication and interpersonal skills.
