Coordinated Care Specialist

We are seeking a dedicated Care Coordinator to be the primary liaison between our clients, their families, and our care team. You will deliver exceptional home care services by efficiently managing schedules, coordinating care plans, and providing compassionate support.

  • Effectively match clients with caregivers based on needs and preferences
  • Coordinate in-home care services, ensuring seamless scheduling and execution
  • Communicate with families, caregivers, and healthcare professionals to ensure optimal client outcomes
  • Monitor and update client care plans to reflect changing needs
  • Ensure compliance with industry standards and best practices for aged care and NDIS
Key Qualifications:
  • Experience in aged care/disability care, preferably with knowledge of NDIS standards and Aged Care Standards
  • Strong organizational and communication skills, with ability to multitask in a fast-paced environment
  • Caring and compassionate attitude, with a passion for delivering high-quality home care services
