Care Coordinator Role

Job Summary:

This role seeks an experienced care coordinator to join our team in the South West region. The successful candidate will work closely with medical practitioners, NGOs and hospital staff to identify individuals in need of palliative care services.

Key Responsibilities:

  • Act as a resource person for staff and volunteers involved in palliative care.
  • Evaluate nursing interventions and recommend effective management strategies.
  • Liaise with healthcare providers to coordinate quality improvement projects related to palliative care delivery.
  • Develop and implement training programs for staff on palliative care practices.
  • Participate in performance management, professional development and training opportunities for staff.

Requirements:

  • A registered nurse with advanced clinical competence and experience relevant to palliative care.
  • Exceptional skills in planning, coordination and decision-making.
  • Strong leadership and membership skills are essential.
  • Excellent communication and interpersonal skills.
  • Ability to incorporate risk management and quality management principles within practice.
  • Current knowledge of legislative and regulatory requirements in Equal Opportunity, Disability Services and Occupational Safety and Health.
