Job Title: Healthcare Professional

This role is ideal for someone who enjoys working with people to improve their lifestyle and remain active and healthy in the community.

About The Role:
  • Coordinate referrals and management of physical activity programs including Falls Prevention, cardiopulmonary rehabilitation and Healthy Lifestyles activities.
  • Facilitate group exercises programs for community members, complete assessments, document and evaluate outcome measures.

The role works closely with the Community Nursing and Physiotherapy teams to provide services across both Forbes and Parkes sites. Education, planning and supervision sessions are conducted together.

Key Responsibilities:
  • Work with clients to develop and implement individualized plans to improve their health and wellbeing.
  • Provide education and support to clients on healthy lifestyle choices and disease prevention.
  • Collaborate with other healthcare professionals to ensure comprehensive care and support for clients.
Requirements:
  • Must be an enrolled nurse with current registration and a willingness to work in a fast-paced environment.
  • Excellent communication and interpersonal skills with the ability to work effectively with clients, families and other healthcare professionals.
  • A commitment to providing high quality patient-centered care that respects individual dignity and autonomy.
