About this Role
We are seeking a dedicated professional to fill the position of Care Partner. This role is an integral part of our community team, responsible for providing support to clients and colleagues alike.
To be successful in this role, candidates must possess the following qualifications:
- A formal tertiary qualification in a relevant field, such as nursing or a related discipline
- Current registration with the relevant professional body, including AHPRA registration as a Registered Nurse
- A national criminal history check and valid Working With Children's Check Card
- A current First Aid and CPR certificate
Candidates should have experience working in the aged care industry, with knowledge of quality improvement principles and standards. Previous experience in a related field is desirable.
The ideal candidate will be responsible for:
- Building strong relationships with clients and their families
- Conducting comprehensive client assessments and developing personalized care plans
- Providing expert health and clinical guidance to home care workers
- Liaising with clients and their families on a regular basis
- Ensuring all practices are aligned with policy and legislative frameworks
