About the Role

As a clinical care coordinator, you will play a pivotal role in delivering high-quality, evidence-based clinical care to adults aged 18 years and over who experience mental health concerns and their families and carers.

Working as part of a multidisciplinary team, your primary focus will be on conducting intake and assessments, including risk assessments, care treatment planning, and providing brief psychological intervention.

  • Your expertise will be utilized to coordinate care with internal and external services, ensuring seamless transitions and comprehensive support for clients.
Requirements
  • You must hold full AHPRA registration as an Occupational Therapist or Mental Health Nurse, General or Provisional AHPRA registration as a Psychologist/Clinical Psychologist, or AASW registration/eligibility as a Social Worker.
  • Flexibility is essential, as you may be required to work afternoons, evenings, weekends, and public holidays.
  • A strong understanding of evidence-based interventions and clinical practice, particularly in the areas of mental health and alcohol and drug use, is crucial.
  • You will possess highly developed communication and interpersonal skills, enabling you to engage effectively with people, advocate on their behalf, and establish meaningful relationships with staff, clients, families, friends, and stakeholders.
