Job Overview

This role is centered around delivering tailored care services to individuals residing in their own homes. It involves assessing clients' needs, developing personalized care plans, and providing ongoing support.

The successful candidate will have a strong background in community aged/disability care, with a focus on service development, coordination, and delivery.

They will be responsible for direct nursing support to clients, including assessments and care planning. A current AHPRA registration as an Enrolled Nurse (EN) or Registered Nurse (RN) is required.

The ideal candidate will possess excellent communication skills, professionalism, and the ability to work effectively within a team environment.

Requirements
  • Minimum of Diploma in Nursing with a current AHPRA registration;
  • Driver's License and comprehensively insured dependable private vehicle;
  • Current National Police Clearance; and
  • Current First Aid & CPR.
Preferred Skills
  • Good understanding of the Government funding environment for aged care (primarily CDC) & NDIS (preferred but not necessary);
  • Good understanding of the hospital discharge environment;
  • Subscribe to a culture of teamwork, caring and quality service;
  • Excellent communication abilities, professionalism, a good reputation & respect for our staff, clients and their families; and
  • It is also essential to have good judgment, quick decision-making, excellent communication, staff management and administrative skills.
