About This Role

We are currently seeking a compassionate and reliable Senior Home Care Worker to join our team. As a key member of our care workforce, you will be responsible for providing high-quality support to our clients in their own homes.

Job Description

This is an exciting opportunity to work with a diverse range of clients and provide personalized care and support to enhance their quality of life. Your role will involve assisting with daily activities, medication management, and promoting independence.

You will work closely with our clients and their families to develop and implement personalized care plans that meet their unique needs and goals. You will also contribute to the planning and coordination of client care from our office, as well as being available for on-call support as needed.

Your dedication and commitment to delivering exceptional care will make a significant difference in the lives of our clients and their loved ones.

Key Responsibilities
  • Providing personal care and assistance with daily living tasks
  • Assisting clients with medication and maintaining accurate records
  • Developing and implementing personalized care plans
  • Maintaining accurate records and reporting any changes or concerns to the team leader
  • Conducting regular reviews and updating care plans as necessary
Requirements

To be successful in this role, you will need to possess:

  • Certificate III in Individual Support (Ageing)
  • Minimum 2 years' experience in a direct care role
  • Strong communication and interpersonal skills
  • A commitment to quality, safety, and continuous improvement
  • A current driver's license and comprehensively insured vehicle
